Please print out and complete each page:

Application for Admission (skip if you already have a LCC ID)

Dual Credit Checklist (skip if you have already taken Dual Credit classes successfully)

Dual Credit Request Form (fill all portions, then print)

Dual Credit Agreement Form

New Student Release of Information Form (to allow parent’s access to records)

Once all forms are completed (with required High School Counselor/Principal’s signature, bring/send in to Dual Credit Coordinator for processing.
